| The following forms plus a COPY of your birth certificate are needed at try-outs: |

| One copy of the SFJOV Try-Out form

One copy of the Rules and Regulations form
|

Two copies of the North County Region (NCR) Membership form
|
| |
One copy of the Medical Release form
|
| |
Download all forms, print them, fill them out, and bring them to try-outs.
Please make sure all forms have your parent's signature. Be sure to include the $10 try-out fee and a copy of your
birth certificate. All this is needed at registration or you will not be allowed to try-out. |

There is scholarship money available,
if needed. Please print out the Confidential Scholarship Assistance form and return to Dawn Berg during try-outs.
These scholarships remain confidential. Amounts available depend on the number of people who apply.
